APPOINTMENTS
All patients of Central Iowa Neurology are seen on a referral basis only. Each patient is required to make an appointment in order to be seen and/or assessed.
If you would like to schedule an appointment, please call us at (515) 255-7414. Our office staff is ready to assist you a in figuring out a time and day that works best for you.
Here are some things to keep in mind before you arrive for your appointment:
- Visit our Forms page to fill out any necessary medical history and other forms required for your first visit.
- Bring any medications that you are currently taking along with you
- If you are unable to keep your appointment, please call us 24 hours in advance to let us know. If not, you may be assessed a "No Show, No Call" fee and potentially dismissed from our practice.
