OFFICE VISITS & PROCEDURES
All patients are seen by appointment. We do our best to see all patients at their scheduled appointment time, and we ask that our patients arrive a few minutes before their appointment to ensure minimal wait times.
We encourage patients to call us with any questions they may have. However, it is important to note that some health-related questions and concerns may warrant an office visit for a thorough assessment and evaluation.
FEES & PAYMENT POLICY
Central Iowa Neurology, PC actively participates with a large number of insurance companies and will submit all payment and billing information forms for all of our patients. Our Financial Responsibility form explains all fees and collection policies in full detail, and responsible party is required to sign this form.
Important information regarding our payment policy:
- You are ultimately responsible for all fees for service, even if you believe they are covered by your insurance provider.
- Fees for completion/production of forms/reports that you request to be sent to yourself, employers, or any third-party are not covered by insurance.
Please note: In order to provide a thorough assessement and give proper treatment, we need to be aware of any dangerous or self-injurious thoughts or behaviors. If not made aware, this can impact your evaluation and treatment plan.
MEDICATION POLICY: CIN does not prescribe opiods or other addictive medications or pharmacuticals.
